This app leverages D3.js, using data from the CDC, for an interactive web map hosted on GitHub where users can explore potential relationships between different health outcomes (i.e., chronic conditions) and health risk behaviors. The app is designed to help communities pinpoint local health challenges, target disparities, understand interconnected needs, guide resource allocations, and set meaningful health goals. I developed the app collaboratively in GitHub with a classmate as part of my Interactive Cartography and Geovizualization course at the University of Wisconsin–Madison.
